Assalamualaikum wbt
SubhanAllah, alhamdulillah, Allahuakhbar!
Being humane.
A simple phrase yet my mind searches for words to describe situations that had to be faced day to day. SubhanAllah.
An elderly women in her 80-s was brought in the hospital as she developed weakness of the left side of her body. She seemed like screaming for words; but she couldn't uttered a word. She nods her head in approval or whenver she acknowledges anything. Relatives by her side were stunned but understood that thir loved one on the bed 'kene angin'.
Her brain was scanned-findings were towards multiple infarct (death of cells in brain due to lack of oxygen). Something common we see everyday. Her condition deteriorated day-to day.
Her family refused for any invasive treatment or procedures.
Being humane.
A simple phrase again. Yet something we find difficult to do in time of stake.
A doctor speaks, "Kalau tak nak ape2, buatpe masuk spital? Balik je la." Family members were stunned. I was stunned too.
My mind keeps mesmerizing upon my late father. A healthy man; suddenly collapsed in the emergency department. Doctor struggled to save his life; but had forgotten or say; not bothered to keep umi updated regarding his condition. She waited outside the ED (emergency department) in despair, fighting in tears and prayed endlessly.
Alast, my late father was intubated (connected to the oxygen machine) to keep him breathing. He was sedated and appeared calm. We nearly failed to recognize him, he looked so different. I had encountered so many patients intubated; struggling with multiple iv lines, with tubes and so on; but i ccould not believe im seeing my dear father in front of me.
His hands were perishingly cold. i watched him gasping and infused with tripple inotropes (to keep his Blood Pressure stable). I watched him returned to Allah peacefully and calm.
I face similar situations everyday in the hospital. SubhanAllah. May Allah gives me strength to face such difficult situations. Death is so near, and yet we can be so laid back with the worldly; which almost blind us all.
Being humane.
We are humans; after all!

Simply THRILLING!!
SALAM TO ALL
SUBHANALLAH, ALHAMDULILLAH, ALLAHUAKHBAR!
Oncalls can be regarded as learning experiences for all of us. We work till we appear worn out; with almost no meals of the day and........sleepless night. Some HOs have really cool nights; with very little amount of admissions and have gooood sleeps. As for me, apparently almost all of my oncalls are eventful; plg tak pun BP drops. Soo Jonah. What can i say; they are worth while experiences and insyaAllah helps me to become a better and well equipped doctor.
My latest on call was a thrilling one. TRULY!! Can only lie on the bed at 6am of the next day. sooo jonah! A pt just came out of OT: had plating of tibia done on him. A 60+year old man, suddenly became unresponsive. BP drops to almost 50/30mmHg. Pupils unequal. A very bad sign!! he was so pale and finding another line was almost impossible. I apologized ti him many times as i had to take blood from his femoral artery. He withdrawn to pain. In view of dropping GCS, we referred him to anest. he was intubated in the ward and sent off to ICU.
In the midst of collapsed pt; there were flooding of lodger cases and admissions. We were out of hands and almost collapsed ourselves. Subhanallah. We suppported each other to move on and be strong as we were worn out ourselves. me, being hypoglycemic rather easily; was nauseating all th way. We didn't have meals of the day; but who are we to complain? This is our work and career and sumtimes we simply have no time to have meals properly :)
As we worked non stop till thnext day; we were called from ICU. The previously intubated pt was bleeding from operative site till soaked the bedsheets! We ran up to ICU and i almost fainted at the sight of splashing blood. Ya Allah...he was bleeding from coagulopathy!!! We reapplied compression bandage to him and his coagulatio profile was corrected with FFPs.
I smiled at my parrtner; who was apparently pregnant at 25 wks. Despite the reckless night; she could still spread that smile on her face and we kept going. I smiled at my MO and she told me that it was the worst on call in history with pts being unstable and flooding cases.

On the nxt morning; we had to continue working despite almost zero adrenaline in our body. I smiled at the sight f my labcoat (how dirty it was) and thinking that it's time for a new labcoat! haha. My tongue was so dry and my mind was exhausted but alhamdulillah llah gave me strength to move on and appreciate the life granted by HIM.
I started my day with bismillah and there you go........doing my rounds in the morning as usual!

Adrenaline Rush!!
Salam to all
Post call is the most relieving period in my career. For me! HEHE.
Adrenaline rush in my veins. Cases of admissions, referrals and lodgers kepp coming. Handphone never stops ringind from almost all wards. Subhanallah..so hectic. But who am i to complain? This is my career, my responsibility and most importantly im doing it for the sake of Allah :)
The most devastating case yesterday was fracure of the femoral bone (tulang paha). This case was referrred from Hosp Setiu. A 3-year old boy could not ambulate and cry upon touching of his right thigh. His right thigh appears deformed and swollen. What do you expect? a fracture....Apparently his brother was influenced by the 'smackdown' usually performed by those fighters in televsison. Apa lagi..dia lompat atas kaki adiknya..at the end..patah kaki adiknya...aduhai...apa nak jadi dengan budak2 skrg ni :(
he was put on skin traction. planned for locking plate of the femur. His femoral bone was fractured and badly displaced..
Lodger cases keep coming in and the last case was up to 4.30am. We called it a day and slept a wnk. haha
It was truly an adrenaline rush experience!! Many cases came in at the same time with us junggling between them..of course priority is priority!
Smile!!!!!

Macam-macam....
Salam to all
SubhanAllah, alhamdulillah, Allahuakhbar!
Approaching 2 months of Orthopedic posting. 2 more months to go, insyaAllah! The first month was pretty blurry for me with lots of things to adjust with. Having patients postponed their operations in regards to late JKSP approval (pinjaman untuk bayaran pembedahan) is quite depressing. Kesian pada patients; stressed out as they have to bear longer hospital stay just because they are in tractions and delayed operations.
Orthopedic on calls are quite challenging and makes your heart pound like mad. (hehe kidding). Why?? Mostly are trauma patients of course...These are some of simple cases i see in the wards during on calls...
.jpg)
A 4 year old girl screamed and cried non stop..Why? She lost the pulp of her index finger. Plus her finger nail :( She thought the running washing machine was merely part of her playground and had her finger stucked in the washing maschine. Her mum managed to pull her hand instantly, but this is the result. The best treatment:? Refashioning of her finger. Or say..MEBO dressing (insyaAllah berkesan juga)
Case 2
.jpg)
A 26 year old man accidentally slipped his finger while carrying heavy loads of concrite. As a result. he alleged nearly amputated finger. (jari hampir terputus). X ray revealed crushed # of distal phalanx right index finger. Treatment? wound debridement +K wire...

This man was referred from Hosp Dungun for nearly amputated rt ring finger. Tapi, his finger was no longer viable. Refashioning was carried out under LA in the ward. He kept that finger (souveniere!!) Scary!!
Macam-macam....but the challenge was a blast!!!! Tiba-tiba minta ortho???
ps: 3 more months of waiting!! :) insyaallah

Perbaharui Niat
Salam to all
SubhanAllah, alhamdulillah, allahuakhbar!
Tersenyum membaca tulisan Ustaz Zul Ramli dalam bukunya U-Turn ke jalan Lurus iaitu: Setiap butir pasir mempunyai hikmah tersirat. SUBHANALLAH...

Something NEW
Salam to all
SubhanAllah, alhamdulillah kulli hal, allahukahbar!
On calls in orthopedic are way challenging; if i am to compare to my previous posting. We almost have sleepless nights but subhanAllah on calls are fun. It is the only time we really get cool opportunities.
Inserting branulas are a norm for HO s. I know it is far more painful than getting your blood sucked out your veins (normal blood taking).

Ward 2CD..memang cool sangat!
Little did i realize how painful it was until i experience hydrocortisone infused through my veins. masyaAllah the pain was quite unbearable in the beginning but alhamdulillah i am used to it, hhuhu. Best gak working in the ward; when staff nurses are your buddies; insyaAllah everything will be fine :) the urticarial reaction had been quite often and disturbing; that i have daily doses of hydrocortisone :)

Example of branula (jarum untuk masuk drip(air)/ubat etc)
However, if you opt for drug infusion; you can simply use butterfly branula for infusion; pahtu cabut. senang cerita, tak perlu simpan branula di tangan. I understand that muslem patients mostly request for branula insertion after isya' prayer (lagi comforting) especially if they are about to go for operation on the next morning.
Experiencing drug infusion on yourself..hmm something NEW :)

How Unpredictable Life Is.....
Salam to all
Subhanallah, alhamdulillah, Allahuakhbar!
"Ingat tak XY, alleged fall dari height tu?" seorang HO A&E bertanya.
"MasyaAllah..." saya tergamam.
Dia di wad semula. T/O dari ICU selepas 2 hari. Kelihatan selesa dan masyaAllah....alhamdulillah.
Saya flashback kembali kejadian semasa saya oncall.
MasyaAllah. How unpredictable life is. Appreciate everything and everyone around you before those moments are gone forever. Live life to the fullest and give your best shot. Allah is ALWAYS with us :)
ps: counting days to go home!! :)

We CAN buy time!
Salam to all
SubhanAllah, alhamdulillah, Allahuakhbar!
ALHAMDULILLAH!
Thank you Allah. If not for this career; i may not experience and realize a lot of things which are new to me. I begin to appreciate life that Allah grants me; as well as reflect on myself based on a lot of experiences.
I admire my specialist. She's not a Muslem but i can read her mind. MasyaAllah; she is a very nice person and understanding as well. May Allah opens her heart towards the beauty of Islam, amin!
An indonesian gentleman; alleged fall from 6 floof heigt sustained severe head injury and multiple fractures. It was fajr; i was bagging him for say almost an hour while waiting ICU call. He was quite restless and i bagged while reflecting to myself. masyaAllah..Allah gives me a chance to live; but sometimes i forget Him. ...but He NEVER forgets me. The beloved never forgets all of us.
Sometimes i miss my Quran recitation; and at times i blame it to time constrains and work. astaghfirullah. Little did i realize that we can buy time if we sacrifice. I should at least sacrifice some of my sleeping period by reciting the Quran; or the non obligatory prayers.
Workloads are not excuses! (an important reminder to myself!)

Allah Lebih Menyayanginya.....
Salam semua
SubhanAllah, alhamdulillah, Allahuakhbar!
"Makcik...(nama)....meninggal.." nafas saya laju. Hati saya menangis.
Amin

Accepting and Learning
Salam to all
subhanAllah, alhamdulillah, allahuakhbar!
SubhanAllah. Thank you Allah for opening my heart towards this beautiful career.
Accepting and learning...
I witness death in front of my eyes almost everyday. I often ponder and reflect on myself,"Cukupkah bekalanku untuk akhirat nanti? Macam mana nasibku di padang mahsyar nanti?"
My patient in acute bed was on ventilator. Went asystole yesterday; CPR done for 5 minutes. Reverted, alhamdulillah. I was extra precautious on that day; checking out her BP; doing frequent chest physio. She has gasping occasionally; that's why frequent suction from tracheal ventilator is needed.
Her IV line kept blocking; if not;;leaking. I searched for perfect vein; and i watched her response. Her eyes open up spontaneously and she pulls her hand away from pain. IV line inserted successfully; alhamdulillah.
Another patient in acute bed has infected sore secondary to prolonged immobilization (breast Ca with bone mets). Her sons never gave up looking after her. Never stopped rubbing her hands and comforting her. Accepting fate and moving on. SubhanAllah.
I learn a lot. About life and death. About how fragile life is. How much we have gained in life that we might have missed the beautiful secret within it.
SubhanAllah. Let's improvise in every aspect; also spiritually. InsyaAllah.
Life is beautiful...if we appreciate it.!!

Allah....Forgive Me When I Whine
Salam to all
SubhanAllah, alhamdulillah, allahuakhbar!
Saya terdengar musik nyanyian arab tajuknya, "Allah..forgive me when i whine". SubhanAllah.
After all...life is no bed of roses!!
Allah....please forgive me when i whine!!!

Wajah Penuh Keibuan -2
salam to all
SubhanAllah, alhamdulillah, allahuakhbar!
Staff nurse mengingatkan doktor on call, "One case; sepsis secondary to infected bedsore nak masuk." Sepsis?? Allah...saya terduduk.

Double Pathlogy?
salam to all
SubhanAllah, alhamdulillah, Allahuakhbar!
Alhamdulillah, my oncall was uneventful. MasyaAllah.
I was browsing through a BHT at the counter; about 8 pm yesterday when suddenly a realtive of a patient ran up to me, " Makcik sakit dada!!"
Ya Allah..
I ran (thanks to my sporty shoes) to the patient. She was tachypneic. "Makcik..tunjuk dengan jari sakit kat mana?" she pinted straight to her epigastric area. Sakit sampai ke mana? She showed her back.
I thought of pancretitis in the first place. but she was sweating, nauseating. Allah...dun tell me she has heart attack?
GTN given. Pain is relieved quite a bit. Blood taken for serum amylase, RBS, FBC, ABG, serum Ca, Cardiac enzymes, LFT stat.
abdome soft, tenderness over epigastrium.
amylase came back with a stunning 5400!! Ya Allah that is sky high raised amylase!!
impression: acute pancreatitis with underlying IHD.
alhamdulillah for quick help of partner on call in blood taking.
This morning, her pain increasing...sent to ICU
May Allah helps her.......amin

A Jonah day...!!
Salam to all
SubhanAllah, alhamdulillah, allahuakhbar!
Such an eventful on call;;a very Jonah one! only one admission, tapi a patient; whom i kad known very well telah kembali ke rahmatullah at 1.10 am. Active resuscitation done; lasted for 2 hours; before family gave in. Did CPR; crying inside but pretended to be strong in the outside.
Went to sleep at 3.30am; but was called by staff nurse about fluctuating BP of acute patients. Branullas being detached and difficult cannulation in chemo pts really are challenging experiences. MasyaAllah thank you Allah for all these tests.
Im counting the days. I kept asking myself whether im ready and prepared for the upcoming challenges. Insyaallah. I'll be entering a NEW chapter of my life in a month time; insyaAllah. Please pray for me......

I Work....for Allah!!
salam to all
subhanAllah, alhamdulillah, allahuakhbar!
What an eventful day..masyaAllah
It all started when i was assigned for all scopies (not the one doing the scopies of course!) this morning. Mula-mula ingatkan ada just few cases...tiba-tiba...
A 50+ year old lady, with LGIB query Ca rectum was supposed to have her colonoscopy done. Tiba-tiba....she collapsed!!! WAAAAA....
Semua panik gak. haha. As for me..my first time being truly involved in a case needing resuscitation. Called blood bank..only have 3 pints blood. Okay...tiba-tiba she bled profusely..waaaa. MasyaAllah. No more blood in blood bank. Okay.,time for new GSH.."Iman..go to blood bank now! Get 6 pints whole blood!" Apa lagi..berlarilah saya!!
I had to run, no doubt people gave me weird stares. Apa tidaknya..me in jubah (haha itulah gunanya pakai seluar panjang kat dalam). I ran like ....tak tahu nak describe, hehe. Went to blood bank and spoke to the staff at the counter..(still panting and catching my breath). Tiba-tiba teringat..oh no i forgot the ice box!!!!!!!!
"Oh no!!" the blood bank and endoscopy room are soooooooo far!
Sambung berlari..dan berlari. Oh Allah...i don't know how to describe this.
MO gave me a call. "Iman...get 6 pints..and 1 cycle TIVC" Oh no..what is TIVC? Okay...i need cryo, platelets and FFP separately. Waaa
MO called again.."Iman..run to OT now. We are pushing the patient into OT..Now!" I was stunned..how to carry 6 pints of blood and the TIVC all by myself??
I ran to OT with hands full of pints of blood with an ice box in another hand. I laughed at myself..Whooaaa i do look funny..panting and running in my jubah. Oh....lucky my sporty shoes help!
Nurses tegur and say..wah..if only doktor boleh pakai tracksuit, sure dah lama awak pakai!" haha. I simply laughed!!
Runnng here and there. Preparing patients for sedation particularly for colonoscopies; i really learnt a lot. Despite those scoldings and people shouting at me urging me to do this and that... i realize that this is a learning process. Mind those shoutings. I'm just a human being; and i'm yearning Allah's love. I work hard for Allah. I help patients as much as i can for the sake of Allah. I got despised and shouted; but i set my mind; i learn because of Allah.
It is sooooo cool if everything we do..IS SIMPLY FOR ALLAH!!!!!! peace!
subhanAllah, alhamdulillah, allahuakhbar!

Wajah Penuh Keibuan
salam to all
Subhanallah, alhamdulillah, allahuakhbar!
HIDUP..MELIHAT, MENTAFSIR, MENGAMBIL PENGAJARAN..
Makcik....Iman rindu makcik!!!

Postcall!!!!!
Assalamualaikum wbt
Subhanallah, alhamdulillah, allahuakhbar!
ALHAMDULILLAH
My first on call was uneventful, alhamdulillah...praise all to Allah. I prayed that no OT pending yesterday...syukur dimakbulkan Allah.
2EF is a cool ward to learn lots of things; particuarly procedures. Alhamdulillah, did desloughing of bedsore and basic wound dressing this morning.
The toughest part in work is breaking bad news. Masyaallah.
What i learned yesterday during on call?
Sifat merendah diri.
Share your knowledge; be as humble as possible.
Subhanallah, alhamdulillah, allahuakhbar!

no JONAH please!!
Salam to all
SubhanAllah, alhamdulillah, allahuakhbar!
alhamdulillah...
After hours in OT without much break; went home with severe backache. Tried to sleep last night but the pain never resolves. Hate taking medication; thus after reciting dhu'a, i dozed off.
Early this morning, the pain got worse. In rukuk; the pain was so unbearable. However, subhanAllah the pain went off completely whenever i do sujud. SUBHANALLAH!
Today i forced myself to take simple PCM for the pain and dizziness. Haha. alhamdulillah.
OOps..tomorrow is my on call-2nd call meaning i'll be in OT again! haha..Better watch out my posture in those long hours standing in OT!
pleas pray that it'll not be a so-called JONAH day!!
amin

OT time!!!
Salam to all
SubhanAllah, alhamdulillah, Allahuakhbar!
ALHAMDULILLAH..
1) Rectal biopsy to rule out Hisrchsprung disease : budak lelaki berumur 4 tahun, mengalami masalah sembelit sejak berumur 2 tahun.
2) Circumcision for recurrent balanitis (berkhatan) : budak lelaki berumur 5 tahun. Sekejap saja khatan ni, hehe.
3) Re excision and axillary clearance: breast cancer (45 tahun)
4) Herniorrhaphy due to L indirect inguinal hernia (50 thn)
SubhanAllah ciptaan Allah memang Maha Hebat.
Mereka menasihatkan saya jangan sombong bila jadi specialist nanti; dan stay the way i am.
Syukur pada Allah atas setiap nikmatnya!!!
It's OT time!! Fuhh :) heheh
Apa kaitan dengan gambar ni?? takde ape2! hehe
SubhanAllah, alhamdulillah, Allahuakhbar!

Humanity

Salam to all
SubhanAllah, alhamdulillah, allahuakhbar!
I learned one important lesson today. Importance of HUMANITY.
Ambil satu ketika untuk meneliti sekeliling anda.
Di sebalik kesibukan anda, jangan sekali-kali melupakan nilai humanity. Saya meneliti pesakit-pesakit saya.
Fahami dan treat mereka 'as a whole'. Moga dengan melihat wajah kita (hehe) sakit pun rasa kurang. (cewah) insyaAllah.
Have humanity. No matter how BUSY you are.
